You will need two different sets of fingerprint cards

Have your fingerprints captured by law enforcement or ScreenID, and printed on a FD-1164 card. You may NOT capture your own fingerprints for submissions.

You may have your fingerprints captured by using ink or a livescan machine and printed onto hard copy FD-1164 cards to be mailed to ScreenID.

Fingerprint cards MUST be mailed to ScreenID for this process.

The reason for two different sets of fingerprint cards?

• If the FBI rejects your first set we can submit the second set at no charge, it’s a complimentary service from the FBI.
• If the second set is also rejected you will have to start the process over from the beginning.
